Hospital Discharge Project

The Health and Social Care Integration Programme aims to provide a streamlined hospital discharge function for residents across North West London. The programme will bring together a number of West London Alliance boroughs in to a single hospital discharge function and will integrate closely with acute providers to achieve a single approach to hospital discharge. The boroughs currently involved in the programme are:

  • Brent
  • Ealing
  • Hammersmith and Fulham
  • Kensington and Chelsea
  • Westminster
  • Hillingdon (Early identification only)

The programme has been working closely with a number of hospital trusts (London North West Healthcare NHS Trust, Imperial Healthcare NHS Trust, Chelsea and Westminster Hospitals NHS Trust and Hillingdon Hospitals NHS Trust) to develop a shared approach to discharge and a joint plan for integrating teams across health and social care.


Phase 1 of the project was implemented between January and June focussing on the 'co-location' of social care staff at a number of different hospital sites. Staff from across the different boroughs have begun working alongside each other and are collectively supporting the hospital wards and Multi-Disciplinary processes within the hospital on behalf of the six boroughs involved. This has allowed greater support across social care teams as well as providing a greater level of support to the hospital sites themselves.


Phase 2 of the project will focus on further alignment of the local authorities, with the co-production of a shared set of protocols and processes across the boroughs and a shared assessment form to be used as part of the discharge. The processes and protocols will be co-designed alongside the hospital team to ensure effective integration with our health colleagues at each hospital site, and with service user representatives to achieve the best outcomes for WLA residents. This period of co-production will culminate in the implementation of a joint hospital discharge function across the boroughs, with local authorities assessing residents and managing discharges on behalf of each other at key hospital sites.
